Transition Updates for Amenda Patients
Hello there!
Thank you for bearing with me as I navigate the many moving pieces involved in getting a private practice up and running. I am actively seeing patients that I was previously seeing at Amenda Clinic. Even if it’s been awhile since our last visit, feel free to reach out if you’d like to start working together again.
These are the steps for establishing care in my private practice for patients I was previously seeing at Amenda:
STEP 1: Complete the Patient Account Setup form
To set up your patient profile in my private practice’s Charm system, please complete the following brief Google Form, which requests the basic information required to set up your account: https://docs.google.com/forms/d/e/1FAIpQLScZ6Gje1eUkmSPxDpE_EJWa8tM8_FzF-lO4HqqnGsyLOF4vCQ/viewform?usp=sharing&ouid=107855760340620473704
STEP 2: Complete your Charm patient portal setup
Within 1-3 days after submitting the Patient Account Setup form, you will receive an email invitation from Curio Integrative Mental Health with the subject line “Patient Portal Registration.” (If you don't receive the email within 3 days, please text me at 503-406-9615.)
Open the email invitation and follow the prompts.
Note: You already have a Charm PHR (Personal Health Record) account through Amenda, so you don’t need to make a new login.
It will ask you to verify your birthdate, sign in with your usual Charm login, and click “submit” when it asks “[your name] will be added to your PHR account with the practice Curio Integrative Mental Health. Do you wish to continue?,” and then Curio will be added as a second practice to your existing account.
Once you’ve completed this process, you’ll see both Amenda Clinic and Curio Integrative Mental Health in your Charm portal and can switch between them as needed.
Let me know once you've completed this step by either texting me at 503-406-9615 or sending me a message through the Charm patient portal.
STEP 3: Get scheduled and complete required forms
Once I know you've completed Step 2, we can schedule a visit.
At that time, I will also send you the forms required to establish care in my private practice. These will be sent to you as Questionnaires through the Charm patient portal.
Please complete and sign all forms before your visit.
Insurance, Sliding Scale, & Transition Options:
Insurance: I am currently in-network with Moda and BCBS.
CareOregon update: I’m in the queue to receive a behavioral health contract, but things are still a little complicated and unclear. If you were a patient of mine at Amenda Clinic and you have CareOregon, please reach out to me directly to discuss options.
Transition Options: Now that’s it’s been 6+ months since leaving Amenda, I’m no longer offering the “transition period” payment option. Please refer to the “Insurance and Fees” section under the “Work With Me” tab for information about my rates, including sliding scale information.

Current Patients: Please direct all communication through your Curio Charm portal or through the methods below:
Primary Patient Communication Number: (503) 406-9615
This is the primary and preferred number for patient communication. This number is used for SMS text messaging and phone calls/voicemail through a platform called Spruce Health and is the most secure option for routine patient contact.

This number is the general business and administrative contact number. It may be used by patients, but it is less secure than the primary patient number listed above and should not be used to share health information.
