Transition Updates for Current Patients

Hello there!

Thank you for bearing with me as I navigate the many moving pieces involved in getting a private practice up and running. I will begin seeing current patients in my private practice the week of January 19th.

Before scheduling in my private practice, current patients will need the following:

  1. Sign an ROI to transfer records from Amenda

  2. Activate your Charm portal for my private practice

  3. Sign updated consent forms 

1. Records Transfer (ROI)

If you have not yet signed the ROI allowing me to transfer your records from Amenda to my private practice, please log into your current Charm portal and complete the form called “ROI - Outgoing Records_ Dr. Wilson” found in Questionnaires and sign it electronically.

2. Charm Portal Activation 

After the ROI is signed, I’ll use your basic demographic information to create a new patient profile for you in my private practice’s Charm system. Then you’ll receive an email invitation from Curio Integrative Mental Health with the subject line “Patient Portal Registration.” I’ll send a brief text from Amenda to let you know when the invitation has been sent.

You already have a Charm PHR (Personal Health Record) account through Amenda, so you don’t need to make a new login. Open the email invitation, verify your birthdate, sign in with your usual Charm login, and click “submit” when it asks “[your name] will be added to your PHR account with the practice Curio Integrative Mental Health. Do you wish to continue?,” and then Curio will be added as a second practice to your existing account.

Once activated, you’ll see both Amenda Clinic and Curio Integrative Mental Health in your Charm portal and can switch between them as needed.

3. Consent Forms

New consent forms for my private practice will be sent through the new Charm portal before your first visit.   

Insurance, Sliding Scale & Transition Options:

To support a more intentional transition, I’m offering a “transition period” payment option for all current patients through the end of February. This time may be used to determine next steps for ongoing care, navigate insurance details, or simply ease the transition. You may have received a letter in your Charm portal with details. If not, please reach out and I can give you more information about this “transition period” rate.

CareOregon update: I’m very happy to say that even though I’m unable to get a contract with CareOregon, I will soon be able to bill them as a non-contracted provider. I’m still in the process of enrolling my practice, but I’m hopeful this will move relatively quickly. Until then, patients with CareOregon may:

  • Request to pay privately using the “transition period” payment option.

  • Pause visits until my enrollment as a non-contracted provider goes through.

  • Switch to an OHP-contracted provider (including most Amenda providers) for continued care with no out-of-pocket cost.

Communication:

As of 1/20/26, I no longer have access to the Amenda Charm portal or my Amenda email. Please direct all communication through your Curio Charm portal or through the methods below:

  • This is the primary and preferred number for patient communication. This number is used for SMS text messaging and phone calls/voicemail through Spruce Health and is the most secure option for routine patient contact.
